EXCELLENCE IN NURSING APPLICATION FORM


  • Grand Rapids Magazine is proud to honor the region’s top nurses in 12 specialties for their extraordinary contributions to health care. This year, we also will honor nursing assistants in an additional category.

    Award finalists and winners will be featured in the July issue of Grand Rapids Magazine.

    Please fill out the following application form to be considered. All application forms will be reviewed by a panel of judges from the nursing profession who will select up to five finalists in each category. From there, the judges will select one winner in each category.

    Nursing categories: Pediatric (neo natal & non neonatal), Emergency Room, Intensive Care, Acute Care, Family Practice/General Medicine, Cardiovascular, Community Health, Oncology, Medical/Surgical, Women’s Health, Palliative Care/Hospice, Mental Health/Behavioral Health, and Nursing Assistant (any specialty).

    Winner profiles will be written based on the application form answers. All winners will be asked to participate in a photoshoot for the Grand Rapids Magazine feature.
